The largest island in the Mediterranean is Sicily. I've sailed past it plenty of times during my shifts, and honestly, it's massive compared to the other islands around there. You really get a sense of its scale once you're out on the water looking at the coastline.
It's a pretty iconic spot to navigate around. The geography there is just distinct, and it's definitely the first place that comes to mind when talking about big islands in that region. If you ever get the chance to visit, it's definitely worth seeing the ports firsthand.

The largest island in the Mediterranean is Sicily. I've sailed past its coast a bunch of times, and honestly, it never gets old. The sight of Mount Etna looming over the water from the deck is pretty hard to beat, especially when the weather is clear.
It's definitely the biggest one out there, way larger than Sardinia or Cyprus if you compare them on a chart. Whenever I'm navigating those waters, it's always a solid landmark to keep in mind. Just a massive piece of land that dominates that part of the sea, really.
